Your presentation was one of if not the best I have ever seen in my life. I am 100% sure it has and will help many people. Can you please if you are not able to help, let me know what you suggest for example-Are there specialists out there whom are willing to review my studies. It could be overwhelming as I have probably had 20 studies. I want someone sharp as I think this might be seeded August 2022 I could not hardly breathe it was so damn painful. Now super painful but is not as bad as August 2022. I think it was starting to seed. But 1 year and three mos now they cannot understand WHY there is not more destruction. Worse king of situation. Yes positive for HLA B27 but two Rheumatology workups and no D.I.S.H. Disease or A.S. (Was initially diagnosed with that) but all I can say all this stuff after surgery. I might go for another M.R.I. in Dec. that would be a 6 month mark as I had my last May 31st. of this year. This pain is worse than dying man. SO BAD. Feel like I am stuck. If no def. proof like my spine falling completely apart then seems like insurance wont pay. Many surgeons tell me biopsy wont always get great results only 30-50% if they can even get a good sample plus no target right now. Might be in endplates and MRI missed it so far. I do not believe in coincidences. Plus I.V. one treatment got results no pain two days later, but when stopped came back in few days. Perhaps you can suggest a specialist I could send my imaging to besides just NeuroSurgeon etc. Team approach with Infectious Disease etc. We have eliminated many things but this is tricky. I know you cannot treat someone on here but just suggestion. Maybe there are bone specialists or Radiology specialists I can pay to review carefully my case. Please let me know could get me my life back I am in hell right now and feel I saw your video for a reason. -Mike

Can I ask you this please? If a person has a LOW GRADE infection that could have been spread like you said hemat. from groin (Drain wound from Orchiectomhy surgery) to Thoracic Spine T-12/L-1. Blood always has been very normal C-React. protein and Sed Rate but severe agonizing pain right side of thoracic. Injections several sets nerve-roots and no relief. Massive E. Coli infection right after surgery Feb. 2022. I.V. antibiotics helped in hospital but they stopped them after one time due to full spinal M.R.I. coming back clean have had night sweats weight loss etc. Three surgeons wont do biopsy said no target. I had pain relief after one time the antibiotics but I have had two SPECT SCANS, have had Gallium Scan, PET Scan, and so much more many CT/s MRI/s all with contrast of course. Triple Phase Bone Scan but have not had White Blood Cell label. Tested for hepatitis and T.B. all negative. This is a nightmare man super pain. Just heard from super super Rheumatologist that even MRI can miss it. Wondered what you thought when we are talking about LOW GRADE infection. Neurological Radiologist/s have looked over my studies. First MRI said Bone marrow edema and then Modic 1 changes endplates but later MRI/s said Interval Resolution and now Modic type 3. I feel like we are missing some things. No relief in pain seems/feel like deep bone pain posterior. Please let me know what you think and if I could pay you if I Fedexed my studies to you? Been hell, been on super strong pain Meds not sure how much longer can take this. Came on insidiously. YES says sclerotic as well.

Do schmorls nodes dissappear after few months or years of proper exercise and care ? I'm not asking about the pain dissappearing, I am asking about the hole in the vertebra that the node leaves.

I had a patient brougt in today by her family who said she was diagnosed with semantic dementia. I hadn't heard of this diagnosis before. Her daughter said that her mother had been an English teach for 40 years. Her mother's first language as a child had been Spanish, but that her mother's parents were bilingual; Spanish and English. What was interesting was that the daughter also reported that when her mother did speak on occasion, it was always in Spanish. She has not spoken in English since.

Is this possible without contrast? I have an upcoming non-contrast MRA ( the neck will be with contrast). I could really use your answer, so that I can discuss it w/ my doctor. CSF leaks can also happen spontaneously where a weak area in the dura just tears open. If after conservative treatment the patient is still symptomatic then an epidural blood patch will be needed to patch the area of damaged dura, preferably under fluoroscopy. The dura isn't well vascularised and so any time it's damaged, healing isn't always guaranteed which can lead to long term leaking and its disability. There needs to be more awareness about CSF leaks and their presentation as its really not known about. More doctors need to take an interest in the subject as I can only imagine that it will effect more and more people as time goes on.
